good morning doctor, i want to do a request that when ever you do case discussion then kindly ask your fellow doctors not to wear face mask as it makes difficult to understand what they are saying . secondly kindly if it is possible then use ppt presentation or other presentation to show on TV screen that what diagnostic test and there value , what you are doing . and the treatment used . so that it makes easy to use to make notes. thanks . I got infected with dengue in 2019 and was hospitalized for 4 days during the course of 4 days of treatment what I genuinely felt is minimal treatment is actually more than enough for Better recovery for me they have given unnecessary treatment this actually put additional stress on me they have given antibiotics and some blood pressure and liver related drugs, antibiotics will not work for a viral disease but still they have prescribed, because of these medicines I got some side effects and bloating and it aggravated my digestive problem, which in turn increased my recovery time, so I feel monitoring the patient and minimal treatment like iv fluids and some vitamin tablets is enough for a patient. Intense treatment is not required unless there is some bleeding or rapid fall of platelets below 30k
Sir in the above case Dengue with Accelerated HTN, if we consider PRES then NTG infusion to control bp is contraindicated as per bmj . Sir what's your opinion and which drug to start if pulse is low. "Nitroglycerine is not recommended in patients with PRES as it may aggravate cerebral oedema.42 Table 1"
Ideal labet /hydra if not available or contra indicated ntg can be given
@@AETCMEmergencyMedicine Thanks sir... Sir since pulse rate is low labet can't be given and hydralazine not available and further NTG aggravates cerebral edema in PRES therefore is it advisable to go with Nifedipine although it has tendency of lowering the BP very fast.
